Cannot Scan Local Storage After Moving Disk

Hi,
 
I use XCP release 1.6.10-61809c with iSCSI FreeNAS.
My FreeNAS was fault two moth ago and i fixed it.
My VDI is not available and VM cannot run after i fixed the freenas, so i move disk to local storage  and VM can run and vdi back to normal.
Yesterday i want to move VDI again to iSCSI but i got error End_of_file and i cannot scan my localstorage.
this is my error log:
 
Error code: SR_BACKEND_FAILURE_40

i check on my LVM that i got error unknown device :

i have done reboot host, forget VBD, Destroy VBD, Forget PBD, Destroy PBD, Forget VDI, Check Duplicate VDI, but it not fixed.
 
I check cache of metadata and i got :

/dev/sdb is use by iSCSI.

i check disk that it use by iSCSI.

i dont know, what should i do to get back my data on VM?
 
after i destroy PBD Local Storage in my XEN Center tell me to upgrade SR Local Storage,
is it Safe?
 
or if i use vgreduce --removemissing, is it safe?
 
i want to get back my data on this VM.
can anyone help me?

Solved.

 

One of My HDD was fail and not detected.

I Clean Harddisk from dust and reattach again.
